All clubs will have now received a link to a Code of Conduct Report form . Rarely are there any issues to report however ES require us as a County to handle any disciplinary matters locally. assess them and escalate as appropriate. I am obliged therefore to gather details as and when required. The form is self-explanatory however I will assist as required.
